Fragrance-Free Policy:
Due to the sensitivity of some of our patients, we ask that all visitors to our office please avoid using or wearing scented products such as perfumes, colognes, lotions, fragrances or essential oils. The fumes from these products may cause some of our patients to suffer adverse reactions. We thank you for your consideration of others and your cooperation with this policy.
Patient Payment & Insurance Policy
We are a private practice and do not accept assignment from any insurance company. Payments are due at time of service and we accept payment by cash, personal check, VISA, MasterCard, American Express and Discover.
We do not accept HMO Insurance, and are not Medicare or MediCal providers.
If you have PPO Insurance, payments are due at the time of service and we will provide a bill with appropriate medical coding that can be sent to your PPO insurance carrier for reimbursement on covered medical services. Office visits are paid in full by the patient. Standard laboratory tests are billed by the performing laboratory directly to your PPO insurance company. Specialty tests are to be paid by the patient at time of service.
