1: Carefully review our policies

2: Download, print and complete the four new patient forms, and bring them with you to your first appointment.

3: Please also bring the following to your first appointment.

  • Photos of all supplements (vitamins, minerals, herbs, etc.) that you are currently taking or have taken in the past three months.
  • Photos of all prescription medications and/or over-the-counter medication that you are currently taking. This includes laxatives, nasal sprays, birth control, etc.
  • Printouts of any laboratory or diagnostic test results that you have from the last 2 years.
  • Your insurance information. While we do not bill insurance directly, we will use your insurance information to prepare a bill with appropriate medical coding that you can submit to your carrier.

Please allow one hour for your initial office visit.


Your first visit will include a comprehensive evaluation that will lay the foundation for finding and treating the cause of your medical condition.
